Damaged Passport

Step 1

Success starts with a carefully prepared Application!

    Every U.S. Citizen, when applying for a U.S. Passport for the first time, must prove two things:

    • They are entitled to U.S. Citizenship
    • They are the person applying for a passport

How to prove you are entitled to a U.S. Passport

    Proof of Citizenship is made by providing one (1) of the following items:

    • Certified Birth Certificate from one of the 50 states
    • Naturalization Certificate
    • Certificate of Citizenship
    • Consular Report of Birth Abroad
    • Citizen Born Abroad Birth Certificate

    If you cannot provide any of the above documents for any reason, contact our office either by phone at 1-800-627-1112 or by using our Online Operator.

    Proof of Identity is made by providing the following:

    • Valid Driver's License
    • Military ID
    • Other State Department approved identification

    Fees for service vary depending on processing time. Please call us for a quote or ask the Online Operator.

Warning: If instructions are not followed exactly as outlined here, your passport could be DELAYED or DENIED.


    An online application with a 2D bar code is required for hand walked applications. All form blanks must be completed. In order to insure that your passport is completed in time for your trip your form must include the following information:

Mail Passport To Section: Should read as follows

    In care of: Will Call International Visa Service
    316 Hammond Dr.
    Atlanta, GA 30328

Permanent Address Section: Permanent address should be your physical home address.

Father and Mother Section: Parent’s Birthday—if you do not know your parent’s birthday put unknown in the blank space. You must complete this section even if your parents are deceased.

Travel Plans Section: You must be traveling within 21 days and have a travel Itinerary to support the answer to this question.

When you have completed the application click on CREATE FORM button at the bottom of the online application form page. (A link to the form appears later in these instructions).

The online form will create a Adobe PDF file which will appear in your browser window. A 2d bar code (see photo on right) will appear in the upper left of the form, which will print on this application.

If there is no bar code on the printed application, your applications will not be accepted.

Go to Step 2

Step 2: Complete and copy the forms

    To complete your application you will need the following:

    You will need the following items:

    1.

    Certified Birth Certificate or a previous passport.

    2.

    Valid driver license.

    3.

    U S. Government form: DS-11 Passport Application and Letter to the passport agency telling how your passport became mutilated.

    4.

    Copy of your itinerary or letter from your company.

    5.

    Two (2) passport photos.

    6.

    Three (3) Client's Letter of Authorization.

    7.

    U.S. Government Fee.

    Next, complete the the following:

    Now, what you must do is:
    Completed

    1.

    Complete DS-11. (DO NOT SIGN THIS FORM).

    2.

    Complete three (3) original Client's Letter of Authorization.

    3.

    Write a check (or purchase a money order) to:
    U. S. Department Of State. The amount will be for: $135.00 (Adult fee) or $120.00 (Child fee, if applicant is under 16 years of age)

    Check must be in the applicant's name. No counter checks will be accepted. Please put the applicant's name and date of birth in the top left-hand corner of the check or money order.

    4.

    Write a check (or purchase money order or have cash) for $25 made payable to either Clerk of Court or U.S. Post Office. (Wait to complete).

    5.

    Travel itinerary or Business letter.

    6.

    Make copies of all the above items. Copy the following items for International Visa Service.

    7.

    Letter explaining how your passport was damaged or mutilated.

    Now, make copies.

    Items to be copied:
    Completed

    1.

    Driver's license.

    2.

    Completed Personal Information Form

    3.

    Proof of citizenship (certified birth record or old passport).

    4.

    Travel Itinerary or Business letter from company

    5.

    Payment to International Visa Service in the form of a check or credit card.

    6.

    Passport Application (DS-11) and statement regarding passport mutilation.

    The copies should be on the OUTSIDE of the SEALED Application Form Envelope.

    DO NOT include the copies or payment to International Visa Service INSIDE the sealed envelope.

    Now, here are the links to forms you will need to complete:

    Go to Step 3

Step 3: Going to a U.S. Government Approved Acceptance Agent.

The Acceptance Agent will complete the official process for your passport application. You will sign the application at the appointed agent office as instructed.

First, find nearest Acceptance Agent to you. Click here to locate an office near you.

What happens at the Acceptance Agent's office?

    The Acceptance Agent will:

    • Have you sign the application DS-11.
    • Record information from your driver's license onto the application.
    • Counter sign the application and affix the official stamp, then place the
      application in an envelope.
    • Seal the envelope and put the official stamps on the outside.
    • Hand the sealed envelope to you, which you will ship to International Visa
      Service.
    Inside the sealed envelope should be:
    Completed

    1.

    Passport Applications signed and completed with 2D Bar Code plus the statement regarding passport mutilation.

    2.

    Certified Birth Certificate or expired passport. (Proof of citizenship), plus the mutilated passport.

    3.

    Two (2) original Client Authorization letters.

    4.

    Employer letter or travel itinerary.

    5.

    Two (2) photos.

    6.

    Check or money order made payable to: The Department of State

    The envelope SEAL must not be tampered with or broken or the application is considered VOID and you will have to start all over agin.

    Go to the fourth and final Step . . .

Step 4: Shipping your documents to International Visa Service.

    Here is a list of what you need to do next. Use the "Completed" column as a final checklist to make sure all documents are complete and enclosed.

    FedEx package should contain:
    Completed

    1.

    Sealed Envelope for the U.S.Passport Service.

    2.

    Copies made for International Visa Service and one (1) original Client Authorization Letter.

    3.

    Personal information form.

    4.

    Payment

    Send the above items via FedEx or other  PRIORITY Next Day Service to:

    Please call International Visa Service at 1-800-627-1112 with the tracking number on the package. OR fax a copy of your personal information form to 404-843-2259. Or you may e-mail a copy of your personal information form to ivs@visalady.com
