Name change by marriage, divorce or court approved name change.

Follow the general Instructions below for Passport requiring a name change by Marriage, Divorce, or Court Approved Name changes (that are within 1 year of passport issue date).

Instructions for passports issued within one year

Send International Visa Service the following:

You will need the following items:
1.	Re-Application Form DS-5504. (Click here to complete the form online).

3.	Two (2) Client's Letter of Authorization.
4.	Certified State or County document which changes the name. Certified Divorce Decree or Certified Marriage Certificate
5.	Two (2) passport photos.
6.	Letter from Employer or travel itinerary.
7.	Check made payable to: Department of State for the amount of $60.

If passport was issued more than one year, use DS-82 form.

NOTE: Certified Divorce paper must state that you are taking back your previous name.

Adding extra Visa Pages to your Valid Passport.



You must use the DS-4085 form only.

Follow the general Instructions below for all passports that need Additional Visa Pages.

USE THE U.S. Passport Re-Application Form for an error on the passport When passport is WITHIN one year from date of issuance:

First you will need to download the DS-4085 PDF form and complete the form. Print out page 4 only, then fill out the form page.

In the section of the application that says:"MAIL PASSPORT TO" and the box "WILL CALL" please type in "WILL CALL INT'L VISA SRVC", and complete the section with your permanent street address.

This form should only be used to add visa pages to a previously issued, currently valid U.S. passport.

There is a $60 fee per application, plus fees for the service you might request of International Visa Service given your travel plan needs.

You will need the following items:
1.	Complete the Additional Visa Pages Form DS-4085. (Click here to complete the online form).
2.	Two (2) Client's Letter of Authorization. Check Option #1.
3.	A check to the Department of State for $60.00.
4.	Letter from Employer concerning travel or travel itinerary.
5.	Two photos.
6.	Your current passport.

Sign all applications and authorizations with exactly the same signature.

On the form in the section:

Mail passport, put the foloowing information:

Instructions for correcting a Valid Passport with an Error

Follow the general Instructions below for a Passport requiring an Error Correction (that are within 1 year of passport issue date).

Send International Visa Service the following:

You will need the following items:

3.	Two (2) Client's Letter of Authorization.
