U.S. Passport Instructions

For U.S. Passport Renewal

You can renew your passport if your passport meets these criteria:

• You were over 16 years old when your passport was issued • Your passport was issued within (15) fifteen years. • Your passport can be submitted with this application. • Your passport is in good condition.

Your passport will arrive in time for your trip if you read and follow these instructions below.

 

What you must do first is:		Completed
1.	Complete Department of State Passport Renewal Form DS-82.
2.	Complete 3 copies of the Client's Letter of Authorization.
3.	Complete the Explanation of Services Form.
4.	Have (2) two passport photos made.
5.	Write a check (or purchase a money order) to: U. S. Department Of State. The amount will be for: $170.00. Please put the applicant's name and date of birth in the top left-hand corner of the check or money order.
6.	Provide travel itinerary or letter from your company.
7.	Submit your current passport.

Send the following to International Visa Service:

You will need the following items:
1.	Application for Passport (DSP-82) renewal form signed and completed.
2.	Two (2) passport photos.
3.	Three (3) Client's Letter of Authorization.
4.	Itinerary or letter from your company.
5.	Personal Information Form
6.	Check made payable to Department of State for $170.00.
Make sure you sign all documents exactly the same, using your full legal name.

All expedited passport applications must now be completed online.

An online application with a 2D bar code is required for hand walked applications. All blanks on the form must be completed.  This form is free. 

In order to insure that your passport is completed in time for your trip your form must include the following information:

Permanent Address Section: Permanent address should be your physical home address.

Father and Mother Section: Parent’s Birthday—if you do not know your parent’s birthday put unknown in the blank space. You must complete this section even if your parents are deceased.

Travel Plans Section: You must be traveling approximately 14 days and have a travel itinerary to support the answer to this question.

When you have completed the application click on CREATE FORM button at the bottom of the online application form page. (A link to the form appears later in these instructions).

The online form will create a Adobe PDF file which will appear in your browser window. A 2D bar code (see photo above on right) will appear in the upper left of the form, which will print on this application.

If there is no bar code on the printed application, your applications will not be accepted.

Children whose passports were issued before their 16th birthday cannot be renewed.
