Clients in the Metro Atlanta area are asked to visit one of our two offices. Here you will experience a professional staff member who can make sure your application is prepared correctly, which will expedite the process.
International Visa Service has its own staff in every major city as well as in many smaller cities who personally hand carry documents to the different agencies, consulates and government offices.
You will need to bring various personal documents which will be required for your particular document application. Please refer to the list below for specific documents to bring. If you do not bring all the necessary personal documents, your application will be delayed.

An online application with a 2D bar code is required for hand walked applications. All form blanks must be completed. In order to insure that your passport is completed in time for your trip your form must include the following information:

Permanent Address Section: Permanent address should be your physical home address.
Father and Mother Section: Parent’s Birthday—if you do not know your parent’s birthday put unknown in the blank space. You must complete this section even if your parents are deceased.
Travel Plans Section: You must be traveling within approximately 14 days and have a travel itinerary or company business letter detailing your travel plans.
When you have completed the application, click on CREATE FORM button at the bottom of the online application form page. Please check all the important information for typos or mistakes. This information will be on the bar code and in your passport, so the form must be perfect.
The online form will create an Adobe PDF file which will appear in your browser window. A 2D bar code (see photo on right) will appear in the upper left of the form, which will print on this application.
Please print two (2) copies of pages 5-6.
If there is no bar code on the printed application, your applications will not be accepted

What to Bring to Our Office . . .
Adults first time passport should bring:
|1.||Certified Birth Certificate.|
|2.||Valid driver's license.|
|3.||Check book, cash or credit card.|
|4.||Applicant must come in person to the office.|
|5.||Social Security Number.|
|6.||Parent's dates and places of birth.|
|7.||If married and/or divorced, bring the dates of those events.|
|8.||Spouse's date and place of birth.|
Children applying for a first-time passport:
|1.||A long form certified birth record.|
|2.||Child under 16 years old must have both parents with them.|
|4.||Social Security Number for the child.|
|5.||Parents must have valid driver's license or passports for I.D.|
For passport renewals:
|1.||The most recent passport.|
|2.||Check book, cash or credit card.|
Photos are included in our prices.
If the name used on the expired passport has changed, please bring the certified court document which makes the change (divorce paper, marriage certificate, official name change).
Children’s passport cannot be renewed. All children need to appear in person with both parents if they are under 14 years of age and one parent if they are 16 years of age. Call us if both parents cannot appear.
All forms contained within this website are free. Although many of them are copyrighted, they can be downloaded and used without charge. Official government forms are for the use of any person, free of charge. International Visa Service does not charge service fees on ANY free government service. Our service fees are in addition to the government fees, based on the time allotted to expedite your documentation needs.
